$35.00 File Set Up Fee Per Family - this fee is only charged the first year

Download initial forms here. These are fillable PDF forms, Complete the forms making sure not to miss the 3 initial areas on page one. Save the file like this:  yourlastname_enrollmentforms.pdf -- once saved, attached the file and email them to me. Once your forms are received, you will be sent payment instructions through Square, a completely safe payment portal. We offer OPEN ENROLLMENT year-round.  It is a very simle process. Do you have more questions, please email the administrator.
